Do higher doses cost more? The short answer
Yes, but only modestly. Moving from the 5 mg pen to the 15 mg pen typically adds somewhere in the order of $100 to $180 a month at the same pharmacy. The dose does not create a separate product with its own fixed national price. It is the same brand and the same pen design in six strengths, and each pharmacy decides what to charge for each one.
That matters because of how people search. Enquiries like "Price for 5 mil", "Mounjaro 12.5mg prices in Melbourne" or "Cheaper price for mounjaro 15mg" assume each dose has a set price you can look up. In practice:
- "5 mil" almost always means the 5 mg pen. Mounjaro strengths are measured in milligrams of tirzepatide, not millilitres, and there is no 5 mL pen. Likewise "5.0 mg" and "5mg" are the same pen.
- A city does not set a price. Two pharmacies a few streets apart in Melbourne, Sydney or Brisbane can quote the same 12.5 mg pen at opposite ends of the range above.
- The cheapest 15 mg price is found by getting the same strength quoted at two or three pharmacies, not by hunting for a special "15 mg" deal. The 15 mg range is the widest in the table, so comparing pays off most at this strength.
The practical rule: always ask for a price on the exact strength written on your script, and compare that strength across pharmacies. A 5 mg quote tells you little about what you will pay once you are on 10 mg or 12.5 mg.
The standard step-up and what 3, 6 and 12 months add up to
Mounjaro is started low and stepped up. The usual pattern is four weeks on 2.5 mg, then a move to 5 mg. After that, if the prescriber wants a stronger effect and side effects are manageable, the dose is increased in 2.5 mg steps, with at least four weeks on each strength before the next move. Not everyone climbs all the way to 15 mg. Many people settle at 5 mg, 7.5 mg or 10 mg because that dose is doing the job, or because stepping up again brings more nausea than it is worth.

A few things to notice. The first three months cost roughly the same whichever path you end up on, because everyone starts at 2.5 mg and 5 mg. The real difference shows up in the second half of the year and depends almost entirely on your maintenance dose. Twelve pens is 48 weeks, a little under a calendar year, so budget a thirteenth pen if you want a true annual figure. And these totals exclude consultations and needles, which are covered next.
Do not buy ahead on a dose you have not reached yet. If your prescriber slows the step-up because of side effects, a 10 mg pen bought in advance can sit unused.
What sits on top of the pen price
The pen is the biggest line item, but it is not the whole monthly cost. When you compare quotes, make sure each one covers the same things.
- Consultation fee for the script. Mounjaro for weight management is a private prescription, so someone has to assess you and write it, and then review you as the dose changes. With a regular GP, part of a standard consultation may attract a Medicare rebate, but any gap fee and the frequency of reviews are up to the practice. Dedicated telehealth weight clinics usually charge their own consultation or program fee, sometimes as a monthly membership, and Medicare generally does not cover these. Ask exactly what the fee includes: initial assessment, dose reviews, repeat scripts, messaging between appointments.
- Telehealth versus GP. Telehealth is often faster to start and may bundle the script, reviews and pharmacy delivery in one price. A GP you already see may be cheaper per visit and can keep your weight management inside your wider medical record. Why the same pen costs different amounts at different chemists
Private medicines in Australia have no set retail price. Each pharmacy pays a wholesale cost and then adds its own margin, dispensing fee and any handling or delivery charge. Several things push the final figure up or down:
- Buying power: large discount chains and online pharmacies often negotiate lower wholesale costs or accept thinner margins than a small independent store.
- Stock holding: a refrigerated product with high demand and uneven supply carries cost and risk for the pharmacy, and some price that in.
- Bundled pricing: a pharmacy partnered with a telehealth clinic may quote a lower pen price because the clinic fee is charged separately, or a higher one because delivery and support are built in.
